Family Income Sources (CDD-801B only)
The Family Income Sources information fields describe the source(s) of a family’s income, regardless if the income sources were used to determine eligibility for subsidized child care services.
Exception: If the family is a Child Protective Services (CPS) referred family and income was not used to determine their eligibility, select “No” as the response to all six of the listed income sources.
Note: The family income sources listed in this section, in conjunction with Temporary Assistance for Needy Families (TANF) and CalWORKs Cash Aid Recipient information field, captures all income sources for a family.
Where to Find It
On the CCD 26 Form, look in the box “Family Income Sources” in Section III: Family Adjusted Gross Monthly Income and Size.
Rules and Guidelines
- This information is required for each income source listed.

There are six income sources:
- Employment
- State-only alien and two-parent programs for CalWORKs recipients
- Housing voucher
- Food stamps
- Other federal cash assistance (e.g., Supplemental Security Income, Social Security)
- Other income source; Note: Include child support payments in this category
Important: Do not include TANF/CalWORKs cash aid recipient information here; this information was collected via a different information field.

For each of the income sources, there are only three choices:
- Yes – The family received income from this source.
- No – The family did not receive income from this source.
- Unavailable – This choice is only available if the family is a CPS-referred family where income was not used to determine eligibility.
