Independent Living Program (ILP) Survey
Starting with FFY 2014, the ILP Annual Narrative Report will be completed by using a survey tool. The data collected
will be used to complete California’s federal Annual Progress and Services Report (APSR) and the federal Children
and Family Services Plan (CFSP), which are required under Title IV-B of the Social Security Act. The information
requested in this report is required by the federal Department of Health and Human Services, Administration on
Children, Youth and Families (ACYF), consistent with provisions specified in Program Instruction (PI)
ACYF-CB-PI-17-05, which requests details associated with each county's ILP and transitional housing programs. For

The survey will be completed online via Survey Monkey. Counties will receive the survey link by receiving an email
from Survey Monkey.
There is no ILP survey currently open at this time.
Foster Parent Recruitment, Retention and Support (FPRRS) Survey
To support this Continuum of Care Reform efforts, time-limited Foster Parent Recruitment, Retention and Support
(FPRRS) funding has been made available to counties for investment in activities to reduce congregate care, to
increase the number of resource families (including relative caregivers), and to retain existing resource families,
as well as to support court dependent minors and nonminor dependents and their caregivers.
Statute requires counties which receive FPRRS funding to report on activities funded via FPRRS during the previous
fiscal year and outcomes achieved from those activities, and to submit a plan outlining proposed uses of FPRRS
funding in the current fiscal year. CDSS is using a SurveyMonkey survey to meet this requirement.
