The CDSS and DHCS are pleased to announce that eight counties or regional collaboratives of counties have been chosen to receive a combined $61.3 million in grant funding to participate in the Children’s Crisis Continuum Pilot Program. The program provides a framework for a highly integrated continuum of care for foster youth experiencing high-acuity needs. The purpose is to fully integrate the system of care for foster youth, thus enabling a seamless transition between service settings and providing stabilization and treatment to youth within the least restrictive setting possible.
The following eight counties and regional collaboratives of counties have received a preliminary award to participate in the pilot program:
- Fresno (regional collaborative with Madera) - $8,500,000
- Los Angeles - $10,000,000
- Monterey - $3,650,000
- Riverside (regional collaborative with San Bernardino) - $10,000,000
- San Diego - $8,500,000
- San Francisco (regional collaborative with Contra Costa, Marin, Solano and Sonoma) - $8,500,000
- San Luis Obispo (regional collaborative with Santa Barbara) - $3,650,000
- Stanislaus (regional collaborative with Merced and San Joaquin) - $8,500,000
Upon finalizing an Agreement with the CDSS, each participant will begin implementation of their Children’s Crisis Continuum Pilot Program.
