IGP RFA 2: New Construction & Major Renovation Program Application Checklist – Child Care Centers
Use this checklist as a guiding document to ensure you have everything needed to submit an application
Project Information
- Total Project Costs - Total project costs include all costs and expenses required to complete a construction project (including pre-development, design costs, and permits) and/or equipment purchases, and other costs associated with emergency mitigation and ADA upgrades.
- Grant Amount Request - Grant amounts restricted to actual project costs based on reasonable bids.
- Annual Child Care Program Operating Budget
- Square footage project site – indoor and outdoor
- Current Capacity – Current licensed capacity and funding categories
- Total expected licensed capacity at new site
- Expected new slots breakdown by age group at new site
- Project Readiness – Construction start and completion date
- Relocating due to federally declared disaster – FEMA proof of federally declared disaster
- Total revenue that would be increased
- Proof of site control – Long-term lease agreement and written approval from property owner for construction/renovations to be performed OR deed, property tax statement, or mortgage payment coupon
- CDSS Child Care facility license for existing site(s)*
- Annual audit, current unaudited financials, or your most current tax return*
- Registration with Secretary of State – Required if the applicant is a non-public entity
- Proof of 501(c)(3) or 501(c)(5) – Required if the applicant is a non-profit, tax-exempt status
- Payee Data Record (STD 204) – Required if the applicant is a non-governmental entity (?)
- Project Design Plan - Architectural renderings or drawings made by contractor
- Bids for Construction/Project Costs – Details on construction bid requirements can be found in the RFA and application*
- Operating Budget, Project Budget, and Grant Budget – Details on budgets can be found in the application*
- Site evaluation documents - Floor plans, building elevations, environmental tests, reports from local or state agencies, notes from site visits with regulatory agencies, etc.
- Proof of reimbursement for work already completed – Details on reimbursement can be found in the application
- Before photos of the project site*
- Other documents pertinent to your project – Details on additional documents can be found in the application

- Narrative description of agency mission/vision/history
- Grant narrative of the project for which you are seeking funding – Narrative shall include a description of the project, impact to the community, etc.
- Narrative description of fundraising and budget activities – 10% match commitment*
