AB434 Document Notification

Assembly Bill 434 State Web Accessibility Compliance Notice

Some documents and forms are currently being remediated to comply with Assembly Bill 434 (AB-434) and are not available from the California Department of Social Services (CDSS) website. To request a copy of a document please email ISD Service Desk with the following information:

  • Email subject: Document Request
  • Email body:
    • Name of document
    • Link (URL) or name of webpage where the document was located, if known.
    • Format of document being requested (Accessible or Standard)
    • Contact name and information for questions

In addition, if you would like the document in an alternative format or to request any other reasonable accommodation, please indicate so in the email.
